Registering a custom message?

How do I go about registering a custom message type?

I'd like to register the next available message type (both 
client->server and server->client) for a message with the following format:

1 U8 message-type (249 ?)
1 U8 sub-type (specific to this extension message)
2 U16 message-length (allows skipping if you don't care about the sub-type)

I'd also like to register one encoding type (-310 ?), to be used for 
turning on this extension.
