unable to resolve host by name: (11001)

You will not receive "unable to resolve host by name" after having entered a
username/password, but beforehand.  The error indicates that the viewer was
unable to lookup the IP address corresponding to the specified hostname, and
so was unable to connect.  This normally suggests a DNS-configuration issue.
