Help with VNC

I am trying to figure out how to connect to VNC at home from my work. On my desktop I have a router and Norton firewall. I realize that the router gives my desktop a IP address for my home network such as 192.168.1.100, but I also have a real IP address. If I put it into my web browser from work, i just get to my router settings page and if i try the same thing but add the :5800 to the end i get a connection refused message. I looked in many message boards and i am still confused. Can someone help me. My email is ctwin530 "at" netscape.com
