connection refused on internet / accepted on intranet

I have vnc installed on a win2000 server. When accessing the server via
intranet i can login using the viewer or the javascript. Works fine. When I
go via the internet to the server (it is on a callback link with an isp) it
displays the javascript asking for the password. It then refuses access with
the "connection refused" message.

Any comments
