Using VNC with external DSL modem

Greetings,
I have an external DSL modem/router.  The modem has an IP of 10.5.2.1.  The
modem is connected to the machine using ethernet cable.  The IP of the
machines NIC card which connects to the DSL modem is assigned by the
providers DHCP server and stays fixed once it is issued.  The machine has a
second NIC card for sharing within the internal network.  I have read the
documentation and can connect to the DSL machine from a client within the
LAN using the IP Address of the internal NIC card which is 192.168.200.254.
However, when I try to access the machine when off site, I get the message
"cannot locate server"  when trying to connect to the IP
address(64.34.254.158) of the NIC card connected to the external DSL modem.
I have also tried connecting to the IP of the DSL modem and get the same
error message.  Any ideas on how this type of configuration should be set
up.
